What is SMART amalgam removal?

Dental amalgam fillings contain mercury as part of their composition. When these fillings are drilled or removed, small amounts of mercury vapour and particulate can be released.

SMART amalgam removal is a structured protocol designed to reduce exposure during this process through additional protective measures. It refers to how amalgam is removed, rather than being a treatment in isolation.

The SMART protocol was developed by the International Academy of Oral Medicine and Toxicology (IAOMT) as a set of recommended precautions during amalgam removal.

At N1 Dental, when an existing amalgam filling needs to be replaced, we follow the SMART removal protocol to reduce exposure to mercury vapour and particulate matter during treatment.

Who this is for

SMART amalgam removal may be recommended when:

  • An existing amalgam filling is failing, cracked, or leaking

  • A tooth requires restorative treatment that involves replacing an amalgam filling

  • A patient wishes to have amalgam fillings replaced using enhanced precautionary measures

SMART removal relates to how amalgam is removed, rather than being a treatment in isolation.

Our approach at N1 Dental

While amalgam fillings can function well for many patients, when replacement is required we choose to apply additional precautionary measures to reduce risk for both patients and the clinical team.

  • Rubber dam isolation to help limit ingestion or inhalation of particles

  • High-volume suction positioned close to the tooth

  • Medical-grade air filtration units designed to reduce airborne contaminants

  • Protective equipment for the patient and clinical team during the procedure

  • Careful handling and disposal of removed material

What to expect

Treatment is carried out in a calm, controlled setting.

  • The tooth is isolated and prepared before removal begins

  • Amalgam is removed carefully using protective protocols

  • The area is cleaned and stabilised

  • A suitable restoration is placed, where planned, or the tooth is prepared for further treatment

Local anaesthetic is used where needed, and our focus is always on patient comfort.

Relationship to other treatments

SMART amalgam removal is not a standalone treatment. It is carried out as part of wider restorative care, planned separately and tailored to the individual tooth.

There is no requirement for amalgam fillings to be removed if they are stable, functioning well and you are comfortable having them.

Decisions about replacement are always based on the condition of the tooth, existing restoration, and the patient's individual circumstances and wishes.

Restoration or primary treatment fees are separate and discussed in advance following assessment.
